Terms and Conditions
Please take the time to read through the information below. This document (a copy of which will be sent to you on email at the time of booking) must be completed and signed prior to the first intake session. Failure to sign the document in time may result in a cancellation of the appointment and removal from the assessment waitlist.
Assessment Procedures
The assessment process consists of 4 stages: intake sessions, testing sessions, report writing period and finally a feedback session.
Intake sessions: These are consultations with the psychologist to discuss the suitability of the assessment and to gather relevant information. Up to 3 intake sessions may be required and is charged at psychologist’s usual rate. For clients below the age of 15yrs old, we ask that parents attend the first intake sessions by themselves. For adult clients with suspected or diagnosed intellectual delay, we ask that you attend the intake with your primary caregiver (i.e. parent or support worker).
Testing sessions: Autism or cognitive assessments require a 2-hour testing session. Other assessments may need observation sessions and a series of questionnaires to be completed.
Report Writing: Up to 8 weeks might be required to complete the report. To minimise delays in report writing, please ensure questionnaires are completed by all relevant parties and returned to the psychologist as soon as possible.
Feedback session: In this session, the results of the assessment will be discussed and explained. Please note, clinicians will not release the report until the feedback session to ensure that results are interpreted accurately and for any concerns answered immediately. For clients below the age of 15 years old, we ask that parents attend the feedback sessions by themselves.
Costs
The costs of the assessment can range from $1,500 to $3,000. The costs can vary due to number of assessments being conducted, the number of intakes required or the complexity of the report.
For NDIS Funded Clients:
NDIS funding can be used to cover the cost of the assessment.
For clients who are plan-managed, we will require a confirmation from your plan manager that there will be enough funding for the assessment.
For clients who are self-managed, please refer to the payment schedule below.
Payment Schedule
Intake and feedback sessions are treated as usual consults and payment will be collected at the end of the session.
Once all the intakes are completed and the testing session has been booked an invoice will be generated with the total cost of the testing session and report (if applicable). A 50% deposit will be required to secure the appointment. The deposit is payable as soon as the invoice is created or 7 days prior to the testing session. If the deposit is not made, the testing session may be rescheduled.
The remaining amount will be collected upon arrival for the testing session.
Cancellations
Intake and feedback sessions are subject to our clinic’s 48-hour cancellation policy.
For testing sessions, cancellations made 7 days or less before the scheduled testing session is subject to a $500 cancellation fee. Exceptions will be made for extenuating and unforeseen circumstances. Please note, the cancellation fee must be paid before another testing session is booked.
Rescheduled appointments maybe 3-12 weeks later as we cannot determine the availability of the psychologist when appointments are rescheduled.
ADHD Assessment
If you are considering medication, please note that our psychologists are not able to prescribe medication. Whilst our psychologist can provide a diagnostic report, it is advised that you contact a psychiatrist who will accept the psychologist’s report and provide a prescription. There is a chance that the psychiatrist will insists on conducting their own assessment.
